Information We Request
The booking form may request name, mobile number, department, preferred doctor, date, period and an optional general note.
Purpose of Use
The information is used only to send an appointment request to reception and allow the team to contact you for confirmation or enquiry response.
How Information Is Sent
When the send button is pressed, WhatsApp opens with a prepared message. It is not sent until the user confirms inside WhatsApp, after which the conversation is subject to WhatsApp policies and clinic procedures.
Sensitive Health Information
Do not enter diagnoses, reports, test results or other sensitive health information in public forms. Details can be discussed through clinic-approved channels.
